REGISTRATION FORM
Please return the completed Registration Form to the Secretariat with appropriate
fees as early as possible to ensure receipt of the conference bag and the
complete set of conference materials upon checking in at the Conference Registration
Desk. Photocopies of the form may be used. Click here
to download forms. Downloaded forms are in pdf format. Please print out the
forms and do as appropriate.
PAYMENT
All payments are to be issued in favour of:

ENTITLEMENTS
Full registrants will be entitled to:

Additional tickets for the Malaysia Night could be purchased
at RM 60 per person
(before 15 July 2003) and RM 80 (after 15 July 2003).
CANCELLATION AND REFUND POLICY
The Secretariat must be notified in writing of all cancellations. Refund will
be made after the Conference. The cancellation policy is as follows:
| Cancellation on and before 15 August 2003 : | 50% refund |
| Cancellation after 15 August 2003 : | Nil |

LETTER OF INVITATION
The Organising Committee will issue a letter of invitation upon request. The
sole purpose of the letter is to facilitate the delegate’s travel and
visa arrangements. The invitation does not imply provision of financial or
other support.
CERTIFICATE OF ATTENDANCE
A certificate of attendance will be issued to all delegates.
LIABILITY
The Organising Committee will not be liable for personal accidents, loss or
damage to private properties of participants during the conference. Participants
should make own arrangements with respect to personal insurance.
